At Haven Psychology Group, we want clients to know how to access their records and where to go with questions or concerns.

How to Request Your Records

If you would like to request a copy of your health care records, please contact our office in writing.

Please include your full name, date of birth, contact information, and a description of the records you are requesting. If you would like records sent to another provider or person, please include that information as well. A signed authorization may be required.

We respond to records requests in accordance with applicable state and federal law.
